To comply with CDSA (Content Delivery & Storage Association - formerly IRMA) anti piracy compliance program requirements, Dynamic Media requires that each new master be accompanied with the Intellectual Property Rights Form to protect copyright owners.

Ontario PST Exemption Form download it now
If you are an Ontario business selling items that you order from us, you can be exempted from paying 8% Ontario retail sales tax by completing and submitting this form with your order. If you don't supply us with this form, PST will be added to your invoice.
This applies to Ontario businesses only. Out-of-province customers will automatically be exempt from Ontario retail sales tax.

Credit Card Payment Authorization Form download it now
Use this fax-back form to make the payment of your 50% deposit using your Visa or Mastercard.
Your deposit may also be paid with your company cheque, money order or bank draft.

In addition to the above forms, your order must of course also include your CD or DVD master disc, and all your artwork files prepared to our specifications.
